🚨 ECG Breakdown 🫀 1️⃣ Normal sinus rhythm (72 bpm) Normal PR, narrow QRS ( 40-60 ms) No AV block, no bundle branch block Normal frontal axis BUT 👇 2️⃣ Anterior leads (V2–V4) show T-wave inversion +/- biphasic morphology extending toward V5. Anterior T-wave inversions + resolved intermittent chest pain = 🚩 This pattern raises concern for: • Anterior ischemia (LAD territory) • Possible Wellens’ pattern (pre-infarction state) ( Possible Wellens pattern) 🔻Type A (biphasic T waves) 🔻Type B (deep symmetric inversions) This ECG sits with Type B wellens ) No ST elevation but that does NOT make it benign. 🛑 Key points If chest pain occurred and now resolved + anterior T-wave inversion 👉🏻 treat as possible evolving ACS until proven otherwise. Needs: • Serial ECGs • Troponins • Cardiology evaluation #MedTwitter #MedX #ECG

TAPVR Classification: 1⃣ Type I (Supracardiac): CPV → vertical vein (VV) → innominate vein → SVC (VV compression can cause PVO). 2⃣ Type II (Cardiac/Intracardiac): CPV drains to coronary sinus (IIa) or directly to RA (IIb). 3⃣ Type III (Infracardiac): CPV → descending VV through diaphragm → portal/hepatic veins or IVC (obstruction common). 4⃣ Type IV (Mixed): PV return splits via ≥2 pathways (high anatomic variability—define with detailed imaging).
