Most gait examinations fail because clinicians try to identify the gait name first. Senior neurologists do the opposite. They ask: What failed during walking? That single shift simplifies gait assessment dramatically. THE 3–5 MINUTE OPD GAIT SYSTEM Forget 20 gait classifications. In busy OPD, assess only 3 moments: WALK TURN STANCE That’s enough to localize most gait disorders. STEP 1 — WALK Ask: Walk till the wall and come back. Now observe only 6 things: • broad base • short steps • asymmetry • foot slap • waddling • hesitation/freezing Do not jump to diagnosis yet. First decide: Is the problem: • weakness? • stiffness? • balance? • freezing? • pain? • fear? STEP 2 — TURN This is the most underused gait maneuver in OPD. Turning exposes hidden brain failure. Watch carefully: • multiple tiny steps → Parkinsonian • freezing during turn → frontal/Parkinsonian • veering → vestibular/cerebellar • fearful slow turn → frailty/fear • dramatic sway without fall → functional overlay A patient may walk almost normally. Turning reveals the truth. STEP 3 — STANCE Now stop the patient. Observe standing posture. • broad base → balance system • stooped posture → Parkinsonian • hyperextended knees → weakness compensation • sway → vestibular/sensory • falls eyes closed → sensory ataxia Then perform only ONE bedside test: Romberg. That’s enough for most OPDs. Now classify the patient into ONLY 5 buckets: WEAKNESS foot slap, toe drag, waddling STIFFNESS scissoring, circumduction BALANCE cerebellar vs sensory vs vestibular FREEZING short shuffling steps, en bloc turn FEAR / FUNCTIONAL excessive caution, inconsistency THE REAL DON’T MISS LIST • sudden broad gait → cerebellar stroke • vertigo + cannot walk → posterior circulation stroke • spastic gait + bladder → myelopathy • early backward falls → PSP • magnetic gait + cognition + urine → NPH • unilateral foot drop → compressive lesion The biggest OPD mistake? Calling every shuffling gait Parkinson disease. Not every shuffling patient has Parkinson disease. Not every broad-based gait is cerebellar. Localization matters more than labels. The real expert question is not: What gait is this?
