Is shopping bag tremor a thing? Spoiler alert: yes it is a rare task-specific condition that can emerge when carrying groceries. Shopping bag tremor is a form of task specific tremor that appears when a weighted object like a bag is carried and it may reflect abnormal muscle activation during sustained posture. Check out this video from Kishav and colleagues that just dropped in Movement Disorders Clinical Practice. Key Points: - Shopping bag tremor emerges during weight bearing and sustained grip rather than at rest. - The pattern frequently overlaps w/ dystonic tremor and may differ from classical essential tremor. - Careful observation during real world tasks can improve diagnosis and selection of treatment. My take: I pay closer attention to task specific tremors then I did earlier in my career. Watching how folks carry a weighted bag can actually reveal brain circuitry problems we may miss in the exam room. Subtle clues matter and they can change management. Occupational and physical therapists are frequently the first to recognize the weight based changes. Here are 5 points that resonated w/ me: 1- Real world tasks can unmask tremor patterns not seen at rest. 2- Dystonia may hide in plain sight during everyday activities. 3- Weight and posture can amplify abnormal motor network signaling. 4- Accurate phenotyping aides health care providers to choose better therapies. 5- Listening to what bothers folks most can guide 'potentially smarter care decisions.' …mentdisorders.onlinelibrary.wiley.com/doi/epdf/10.10… @ParkinsonDotOrg @FixelInstitute @movedisorder @MDCP_Journal @tremorjournal
