One of my favorite takeaways from @CameronLMcCord was how deeply the navy looks at what actually caused something to go wrong. Sometimes it’s not a broken process, it’s that someone got zero sleep because of a noisy fan above their bunk. I loved that framing because in tech we usually jump to “work harder,” “hire smarter,” or “add a workflow,” when sometimes the better question is: was the human actually set up to succeed?
