@AvrahamCooperMD Super interesting! I wonder in the positive anticoagulation covid-19 studies whether the benefit of heparin in early disease may be partly due to this diuretic effect. It’s surprising how much non-cardiogenic pulmonary covid patients have that responds well to lasix.

@MKIttlesonMD Also “daily labs” that won’t change your management should be stopped. If the answer can wait to the next day, save your patient an extra phlebotomy stick and consolidate your labs.

@MKIttlesonMD Patients sometimes hear what they want to hear so they assume they’re going a procedure when they see the specialist. Better to give our colleagues the benefit of the doubt before assuming they’ve set up unrealistic expectations.

@Bob_Wachter We have to treat inbox messaging as a limited resource. Charge by the message, monthly/yearly message allotments. Borrow the playbook from the cellular carriers in the pre-unlimited days when plans had message and minutes allotments.

@nickmmark @LancetRespirMed Yes I agree no support of full dose anticoagulation for ICU patients. I was wondering about non-intubated. COV-BARRIER trial specifically excluded intubated patients on admission. Would you still extrapolate the data to these patients?

@charlesornstein This would be an acceptable approach if we could get their parents and older siblings to accept vaccination to protect those not eligible for vaccination, but unfortunately that isn’t a safe assumption in American communities.
